The FX5U-32MR/DS is a Mitsubishi Electric MELSEC iQ-F series CPU module — the FX5U standard type with 16 inputs / 16 outputs, relay outputs, powered from 24 V DC, and with removable screw terminal blocks (M3 screws). What does the /DS suffix mean?

It denotes a 24 V DC power supply version. The module takes 24 V DC with a +20% / −15% tolerance and needs no AC feed, which suits OEM panels that already distribute 24 V DC.

Is this really a relay-output module on a DC supply?

Yes. The official specification lists relay outputs, 2 A per point with a maximum of 8 A per common terminal across 4 points, identical to the AC-supplied FX5U-32MR/ES. Only the supply and the supply circuitry differ.

Transistor or relay for my application?

Relay for heavier or AC-switched loads and mechanical isolation; transistor for fast switching, pulse outputs and positioning. The FX5U-32MT/ES is the transistor equivalent, but note it is an AC-supply model.
